NMHC has contracts with over 54,000 pharmacies throughout the United States, the Virgin
Islands, Guam and Puerto Rico, providing easy access for members in all locations to fill
prescriptions. We are the #1 ranked independent PBM in Retail Pharmacy Network in the 2004
PBMI Customer Satisfaction Survey.

All pharmacies within the NMHC network are connected to our online claims adjudication system
and clinical pharmacy interventions. In addition, they are all contractually bound to the
reimbursement formulas offered by NMHC at the time a client contract is executed. All comply
with Maximum Allowable Cost (MAC) pricing limits and accept the lower of the NMHC MAC price
or the negotiated discount off the generic actually dispensed.

Clients typically choose between two retail pharmacy network options: Preferred or Exclusive.
The Preferred network includes all pharmacies that participate with NMHC. Members may use any
network pharmacy with this option. The Exclusive network is tailored to the client’s needs and
requirements. Members are limited to certain specified pharmacies, and discounts tend to be one
or two percentage points higher, but negotiated discounts are typically higher.

| Credentialing Network Pharmacies |

- NMHC assesses the qualifications of every pharmacy in our provider network. Our credentialing
process verifies that pharmacies meet certain requirements and identifies those that provide
specialized services. Areas assessed include:

- Online claims process abilities
- Support of NMHC client specific plan design
- Current license & registration of the pharmacy
- Willingness to accept negotiated discounts
- Appropriate amounts of professional liability insurance
- Management of Disease Intervention program
- Regulatory agency disciplinary action reports
- Random pharmacy audits
