| Annual Service Review |

An annual NMHC Service Review provides clients information on the performance levels of every
department within NMHC. This report provides statistics and detailed information on the many
services NMHC provides including turnaround times on plan changes, handling of eligibility files,
accessibility of the pharmacy network to members, prior authorizations and other services. The
Service Review also provides industry benchmarks allowing clients to evaluate NMHC’s services
on an objective basis.

| INFO — Online Client Access |

NMHC provides a secure web-based program, called INFO, allowing clients to gain real-time
control over their benefit plan and member eligibility. INFO is used to review information on an
individual member, physician or pharmacy. It is also used to maintain member eligibility
(additions/deletions), check claims status (paid/pending/denied) and make adjustments to the
benefit plan. In addition, clients may perform patient medication overrides to authorize multiple
refills of a plan-restricted or excluded medication. This flexibility allows the client to meet the
special needs of individual members as well as view, add, terminate or reinstate medical exceptions.
